Initial Psychiatric Intake

Service Description

Please Note: Prior to completing an Intake for Psychiatry you must have an established Therapist. You can schedule an appointment to do a visit with one of our Therapists at the same time that you schedule a Psychiatry Appointment. Appointments with Therapists may be covered by your Insurance. For questions call or text our Front Desk at 239-354-7187 Prior to your appointment you will be asked to sign up for our Patient Portal, where you will be able to complete your paperwork and a preliminary patient History Form. You will also need to complete a brief Depression/Anxiety Assessment. This will give the Clinician an in depth understanding of your situation prior to the visit. On the day of the appointment you will be asked to come in 10-15mins early, to complete your vitals. This will ensure that you can take advantage of the full session time with the Clinician. Once the Intake is complete you will be prescribed medication and a follow up visit will be scheduled. Follow up visits that don't require blood work can be scheduled via Telehealth or in person.
